FLU CLINIC 2022
The Arkansas Department of Health will be coming to our school to give students their annual flu vaccines on October 5, 2022. The Health Department will not have Flumist available this year. NO FLUMIST
The consent forms will be sent home only if requested by filling the bottom portion of this letter and sending back to school ASAP. The school will need the consent form filled out on the front and back by Tuesday, October 4, 2022. *The CONSENT FORM must be filled out entirely if your child is to receive a vaccine!* If you have questions or concerns please contact the nursing staff at 641-1023.
